We understand that the Midwest Junior Golf Tour tournament coordinators
and course owners are not liable for any accident or injuries suffered while
playing in sponsored events. By

from any damages by us or our child while participating in, or traveling to or
from a sponsored tournament.

MEMBERSHIP FEES: There will be an annual membership fee of $25 per member.
PLEASE NOTE: Payment of an entry fee for a tournament does not guarantee
entry into any tournament. All tournaments are first come, first served. Please
send in your entry forms before the printed deadline on each tournament entry
form.
PAYMENT INFORMATION: Please make all checks payable to Midwest Junior Golf Tour
